Pramipexole
Reviewed Nov 2026FDA label
MirapexDopamine agonist
- Forms
- IR: 0.125, 0.25, 0.5, 0.75, 1, 1.5mg | ER: 0.375-4.5mg
- Dose
- Start 0.125mg TID, titrate weekly to 1.5-4.5mg/day
- MOA
- D2/D3 receptor agonism
Indications
- Treatment of the signs and symptoms of idiopathic Parkinson's disease, effective in both early disease (as monotherapy) and advanced disease (as an adjunct to levodopa)FDA-labeled
Mechanism of Action
- Pramipexole is a non-ergot dopamine agonist with high in vitro specificity and affinity for the D2 subfamily of dopamine receptors, binding preferentially to D3 receptor subtypes
- Stimulates striatal dopamine receptors, mimicking endogenous dopamine's action to alleviate parkinsonian motor symptoms
- Not extensively metabolized; eliminated largely unchanged renally (~90% urinary excretion); terminal half-life ~8 hours in younger patients, ~12 hours in the elderly
Full Dosing
Adult / usual
- Titration schedule: Week 1 — 0.375 mg/day; Week 2 — 0.75 mg/day; Week 3 — 1.5 mg/day; Week 4 — 2.25 mg/day; Week 5 — 3.0 mg/day; Week 6 — 3.75 mg/day; Week 7 — 4.5 mg/day
- Effective maintenance range: 1.5–4.5 mg/day in 3 divided doses; maximum recommended dose 4.5 mg/day (1.5 mg three times daily)
- Discontinue gradually (taper over 1 week) rather than abruptly to reduce risk of withdrawal symptoms/hyperpyrexia
Renal
- Immediate-release, normal renal function (CrCl >60 mL/min): start 0.375 mg/day in 3 divided doses; increase no more frequently than every 5–7 days
- Renal impairment dose adjustment (immediate-release): CrCl 35–59 mL/min — start 0.125 mg twice daily, max 1.5 mg twice daily; CrCl 15–34 mL/min — start 0.125 mg once daily, max 1.5 mg once daily; CrCl <15 mL/min/hemodialysis — not studied
Contraindications
- Known hypersensitivity to pramipexole or any component of the formulation
Drug Interactions
- Cimetidine: inhibits renal tubular secretion, increasing pramipexole AUC by ~50% and half-life by ~40%
- Carbidopa-levodopa: no effect on pramipexole kinetics; levodopa Cmax increased ~40% and Tmax shortened when co-administered
- Amantadine: may slightly decrease pramipexole clearance
- Dopamine antagonists (e.g., antipsychotics, metoclopramide): may diminish pramipexole efficacy
- Other drugs eliminated by renal tubular secretion (e.g., diltiazem, verapamil, quinidine, quinine, triamterene): may reduce pramipexole clearance
Adverse Effects
- Impulse control/compulsive behaviors: pathological gambling, hypersexuality, compulsive eating (including binge eating), and compulsive shopping — generally reversible with dose reduction or discontinuation
- Somnolence and sudden onset of sleep during activities of daily living, including driving, sometimes without warning signs; more common at doses >1.5 mg/day
- Orthostatic hypotension, especially during dose escalation
- Hallucinations — early PD 9% vs 2.6% placebo; advanced PD 16.5% vs 3.8% placebo; risk ~6.8-fold higher in patients over 65
- Dyskinesia and exacerbation of pre-existing dyskinesia (advanced disease)
- Nausea, dizziness, insomnia, constipation, asthenia (early disease)
Monitoring Parameters
- Continually reassess for daytime drowsiness/sudden sleep episodes, particularly regarding driving safety
- Blood pressure monitoring for orthostatic hypotension, especially during dose escalation
- Direct, specific screening of patient and caregiver for new or increased gambling, sexual, spending, or eating urges at each visit
- Monitor for hallucinations/psychosis, particularly in patients over 65
- Renal function at baseline, with dose adjustment for impairment
- Periodic dermatologic skin examination for melanoma (increased incidence observed in Parkinson's disease population)
Pregnancy & Lactation
- Limited human pregnancy data; animal studies showed embryo resorption and inhibited implantation at clinically relevant doses in rats — use only if potential benefit justifies potential fetal risk
- Not known whether pramipexole is excreted in human milk; animal data show drug concentrated in milk 3–6x plasma levels and pramipexole inhibits prolactin secretion, which may impair lactation — decide between discontinuing nursing or discontinuing the drug
